Use the Expense Reimbursement Form to request a reimbursement for approved expenses you incurred on behalf of the guild. Print, fill out, photograph/scan, and email the pdf form. Email completed forms (with attached receipts) to treasurer@southsoundmqg.org.
Community Quilts Donation Tag
Do you have a quilt to donate? Please print and fill out this Donation Tag and attach it to your quilt. Then drop off your quilt at Patti's house in the bins on her porch (or contact Patti to make other arrangements). Remember to attach a separate tag for each quilt you donate. See the "Community Quilts" page for more information.
The Charitable Donation Acknowledgement Form is used to request a signed acknowledgement from SSMQG for a charitable contribution you made to the guild. Your contribution can be in the form of cash, goods, or services. The dollar value of your contribution must appear on the form as well as a description of the item/service. Send the form to the guild treasurer at treasurer@southsoundmqg.org for recording and signature - the treasurer will return a signed acknowledgement to you.
